that is one of the KASC (konami arcade style controller, similar to their arcade style IIDX or Pop'n music controllers) DDR pads. did you miss the KONAMI at the bottom of the pad

i don't remember when they were released but it was a long time ago and very few were made (like, i think a few hundred at most)
