Last December, I purchased a BNS metal pad off of ebay. The entire thing was still in it's original packaging when it was sent to me. The only thing that might have been touched or used was the control box, which looked repackaged. The pad worked fine for about a month, working flawlessly with Extreme (PS2), Ultramix (with PS2->Xbox converter), and Stepmania (PS2->Xbox->USB). Then it started to have problems with Stepmania and Ultramix. I tested the converters and each was working properly. The pad still worked with my PS2. In a week, it died.

The lights on the arrows still light up when pressed, but they don't register on the PS2 (in any game). It isn't recognized as a gamepad anymore on my PC.

Is this a control box problem? Or is this the pad? How can I tell and what can I do about it?

there aren't any electronics in the pad that can actually break. For argument's sake, it might as well be 4 sets of each a pair of sheet metal and wires. Its definately the control box. Id suggest replacing the box or soldering to a controller (cheap PSX controllers worked for me)

It can't be a BNS Metal Pad, I have one and it does not have any lights on the arrows...

I'll put a picture of it up, but are you sure yours is a BNS? There's been a bunch of people that have recieved a "blueshark" pad (?) when they ordered a BNS.
