I want to collet information about DDR players so we can finally see if there is any coorilation between the player and their performance and form nice pretty graphs, tables and pie charts of all the information that keeps getting asked in DDR communities everwhere.
I'm wondering what questions I should have on my DDR survey. I want to have a short and a long form, since there is important and less important information for me to know.
EDIT: to clarify I'm asking "what other questions should I add to my survey?"

So far this is what I have:
short:
identifier
gender
age
height
weight
level at which you can easily pass/get good grades on
level at which you can just barely pass
how long playing?
long:
identifier
first letter of first name
gender
age
height
weight
lost weight?
at what level can you: just barely pass?
easily pass/get good grades on?
AAA song of what level?
which of 10 foots can you barely pass?
which of 10 foots can you B or better?
which of 10 foots can you AA or better?
which of these onis have you completed?
double skill questions (just like the single ones)
do you play mainly at home/in arcade?
did someone to teach you or did you learn by yourself?
how long playing?
Do you compete?
PA?
Freestyle?
Do you play other video games regularly?

What statistics do you think that I've missed and should include?

level at which you can just barely pass: I can pass all

lost weight? 20
at what level can you: just barely pass? I pass all
easily pass/get good grades on? 8 footers
AAA song of what level? 8 footers
which of 10 foots can you barely pass? Bag
which of 10 foots can you B or better? Max and Maxx
which of 10 foots can you AA or better? none
which of these onis have you completed? I have completed all but 4
double skill questions (just like the single ones)

I'm looking for questions right now, not people's answers yet. I'm going to make a web form so I can collect the data in a nice orderly fashion. I want to make sure that I have all the information that should be collected first.

Speed preferance?
Is there something your known for doing that no one else actually does while playing?
Specialty songs?
Prefered Mods? ( ex: 1.5 speed, dark, boost, ect.) _________________
